What personal data we collect and why we collect it
Comments
When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
Media
If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.
Cookies
If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.

How long we retain your data
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.

All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.

HEALTH CHECK FORM
By clicking the box on the check out page you agree that you have read and submitted the following health check form. You agree you have no medical issues that can cause any health related problems when carrying out our exercises or after.
You have answered “Yes” to the following question.
- Have you been cleared by your doctor to begin an exercise program?*
You have answered “No” to the following questions.
- Do you ever experience unexplained pains or discomfort in your chest at rest or during
physical activity/exercise? - Has your medical practitioner ever told you that you have a heart condition or have you ever suffered a stroke?
- Do you ever feel faint, dizzy or lose balance during physical activity/exercise?
- Have you had an asthma attack requiring immediate medical attention at any time over the
last 12 months? - If you have diabetes (type 1 or 2) have you had trouble controlling your blood sugar
(glucose) in the last 3 months? - Do you have any other conditions that may require special consideration for you to
exercise? - Are you currently suffering from any injuries that may limit your training?
FEES AND PAYMENT
Contract Duration and Automatic Renewal: This contract, NJs Fitness Studio Membership | Weekly, is in force as of the day the customer signed up for the membership, with automatic renewal for subsequent periods, unless cancelled by either or, with notification of 30 days in advance of the next billing date.
Agreement to Pay Recurring Fees:
By clicking the box on the check out page you agree that you have read and agree to the following:
(a) I agree to pay 6 weekly recurring fees as stated in the membership plan of NJs Fitness Studio Membership | 6 weekly plan selected by me, autopay as follows via Client authorized automatic credit card or checking account ACH charges on a weekly basis until I notify NJs Fitness Studio in writing that I would like to terminate. I will submit this request 30 days prior to my upcoming billing date
(b) I hereby certify that I am the holder of the credit card, or an authorized signer on the bank checking account detailed below.
(c) I understand that I will be notified if my credit card or checking account payment fails to authorize for any reason, and that a $20 late fee will apply if I do not provide a valid credit card within 3 calendar days of the original rejection date.
(d) I understand that my service will be deactivated if my account becomes more than 10 calendar days late
(e) I understand I will automatically be charged any bank fee imposed on the clubs, plus a processing fee of up to $20, for any returned payment item due to closed accounts, insufficient funds etc.
(f) I understand that if I dispute any of the above and that my actions require the employ of an attorney, I agree to pay all attorneys fee and cost associated with any action to collect this obligation.
